Keep All - These are among 22 subcats by nationality in Category:Physiotherapists. Our standard practice over the years has always been to exempt small subcats like these from automatic merging or deletion.

As a matter of fact, this practice was alluded to in the long-standing former guideline WP:SMALLCAT, which said "unless such categories are part of a large overall accepted sub-categorization scheme." And it's also referred to directly in the Essay at WP:MFN (which is a work-in-progress for a guideline to supersede WP:SMALLCAT), as follows:

6. The "large overall accepted sub-categorization scheme"

The "large established tree" clause exists so that categorizing editors can add articles to categories without checking beforehand whether that category exists or should exist. Because it is part of an established tree they may rely on the fact that it exists or should exist.

Support lowercase as these are all plurals and thus descriptions, not titles. While MOS:JOBTITLES doesn't explicitly apply to category names, there's also nothing that forbids it either.
